This report may contain information about your character, general reputation, personal characteristics, mode of living, criminal records, civil records, and other public records.

I understand that this authorization remains on file and may be used for subsequent screenings during my volunteer tenure, if applicable.

I consent to receive any pre-adverse action notice and final adverse action notice required by the Fair Credit Reporting Act electronically, at the email address I provide, rather than by paper mail. I understand I may request a paper copy of any such notice at no charge by contacting support@screenforgelabs.com, and that I may withdraw this consent to electronic delivery at any time by the same method, without affecting the validity of any notice already sent. I have the hardware and software (an email account and a PDF-capable device) needed to receive and retain these notices.

As part of this screening, I also agree to verify my identity using a government-issued photo ID and a facial-liveness selfie through our verification partner, Didit. This confirms I am who I say I am, ties my identity to this application, and helps protect the people I will serve. I consent to the collection and processing of this information (including biometric data) for identity verification and fraud prevention, as described in the Privacy Policy.
